OUR SERVICES INCLUDE:

Individualized Care Planning: We develop a Plan of Care, taking into account individual and family preferences. The goal is to implement a service plan that allows the client to achieve the highest quality of life in the least restrictive setting. A thorough nursing assessment, including a bio-psycho-social and safety assessment, determine the care needs.

Medical and Cognitive Monitoring: Monitoring a client's ability to manage activities of daily living, combined with on-going nursing assessments of medical conditions and mental status, relieves family members who live at a distance. Accompanying the client to medical appointments, in order to interact with a client's personal physician, helps support the treatment plan recommended by that physician.

Placement Assistance: We provide information and guidance for families who need alternative living situations for older adults. Assistance in implementing transfers to Rehabilitation Facilities, Assisted Living, quality Nursing Homes, and hospital-based Hospices are typical family requests.

Medical Management and Monitoring: An important aspect of health maintenance is establishing an individualized medical management system to improve compliance in taking medications in a way that maximizes effectiveness.

Referrals: We can recommend other specialists that will provide assistance to your loved one, such as Geriatricians and other medical specialists, Financial Planners, Estate Attorneys, and Elder Law Attorneys. We can make referrals for Hospice care. We can also help you connect with state and federal funding sources for home health care needs, pharmaceutical programs and transportation needs. Respite Programs for caregivers can provide needed relief.
